Your First Visit & Patient Forms

We’re happy to welcome patients of all ages—from kids to seniors—and we understand that dental appointments can make anyone feel a little anxious. If you’re feeling nervous or have concerns, please let us know! Our team is here to answer your questions and help you feel comfortable every step of the way.

What to Expect

After we receive your referral, our front desk team will contact you to schedule your appointment. During this call, you can discuss treatment options, fees, and share any important health details. While your full medical history will be included in your registration packet, sharing key information upfront can help us prepare for your visit.


Some treatments—such as wisdom teeth removal, dental implants, biopsies, or multiple extractions—require a consultation with the doctor before the procedure. For simpler extractions (two teeth or fewer), you can choose to combine the consultation and procedure in one visit, or schedule them separately.


If you’re a new patient, you’ll need to complete our registration packet, which includes your personal details, medical history, and privacy practices (HIPAA). You can:


  • Request the forms by email
  • Download and print them from the link below
  • Arrive 10–15 minutes early to fill them out in the office


Please bring a current list of medications and your dental insurance card. If your appointment involves a biopsy or pathology, we’ll also need your medical insurance information.
