Please note that our practice is
out of network with all insurance providers. However, we are happy to submit a claim to your insurance company after your visit (Medicaid and Medicare supplements are excluded). Payment is required at the time of service, and your insurance will reimburse you directly. During scheduling, we can review estimated costs, but exact fees can only be determined after the doctor evaluates your case and creates a treatment plan.
